Prescription Assistance Program of FUMC – Rockport
A ministry to assist eligible individuals obtain free or low-cost prescription drugs. Eligible individuals must not have Medicaid or other prescription drug coverage.*
Hours of Operation: Mondays from 1:00 – 4:00 P.M.

Please bring the following to each visit:
- Medication bottles or prescription(s)
- Drivers License
- Social Security Card
- Income Information
- Supplemented Insurance Cards
- Medicaid Denied Letter
- Pharmacy Print-out
* There may be assistance from some of the Pharmaceutical Companies
for individuals on Medicare Part D (Prescription Coverage) who are in the "do-nut hole".
